Lifescripts Practice and Patient Resources
The Australian Government Department of Health and Ageing funded the development of a range of resources to assist general practice to build on preventive activities being undertaken in the primary health care system. Lifescripts provides general practice with tools to assist patients to make healthier lifestyle choices. The consortium comprised the Victorian Council on Fitness and General Health (VICFIT); the Centre for General Practice Integration Studies, University of NSW; the National Heart Foundation of Australia (NSW Division); School of Health Sciences, University of Newcastle; Department of General Practice, Flinders University; and Southcity GP Services.
The Lifescripts resources developed include:

- Checklist, Flyers*, Poster*
- Assessment guidelines
- Assessment tools
- Prescription pads
- Medical record summary stickers
- Practice manual
- CD Rom (includes motivational interviewing component)
(*Including versions designed for Aboriginal and Torres Strait Islander patients).
The Practice Manual reccommends a range of additional patient resources for each risk factor.
