The Department of Health and Ageing is currently calling for Expressions of Interest (EOIs) from Divisions of General Practice to participate in the Lifescripts Demonstration Divisions project.  Successful Divisions will be funded to implement strategies with selected practices which assist them to incorporate Lifescripts and related lifestyle risk factor management into their activities. Demonstration Divisions will be required to collate, disseminate and share learnings/experiences through participation in an ongoing national review of project activities.

EOIs will need to be complete and submitted by 21st of May 2007.

Divisions wishing to participate in the Lifescripts Demonstration Division project may choose to submit a proposal from a single Division, or a consortium with other Divisions of General Practice, State Based Organisations, local community organisations and/or non-government organisations.  The Division must be the lead agency of the consortium.

The Department is seeking to fund between 8 and 12 Demonstration Divisions.  To ensure that information is gathered from a range of settings and practices the Department will seek to fund:

• Demonstration Divisions in a number of States/Territories, in order to include practices in a range of locations (rural, remote and urban)
• a mix of strategies to support practices, including those that involve practice nurses and those that utilise local referral pathways; and
• a range of approaches that enable information to be collected on implementation in:
  • both large and small practices
  • some practices with Aboriginal and Torres Strait Islander patients.
