Hornsby Ku-ring-gai Ryde Division of General Practice
The Division will apply a model based upon the National Primary Care Collaboratives methodology, in at least eight practices, to determine activities which will increase the use of Lifescripts by General Practitioners. The Division will run workshops, and assist each practice to apply the plan, do, study, act (PDSA) cycle. Following each workshop the Division will collect data on the usage of Lifescripts, and analyse the effect against the PDSAs implemented by each participating practice. The workshops will focus on identifying current barriers and enablers to Lifescripts utilisation, engendering patient engagement and the integration of electronic software into lifestyle risk management.
Knox Division of General Practice & Eastern Ranges GP Association
The aim of these two Divisions is to implement a model using Health Coaching principles within the General Practice setting. Practice Nurses and GPs from at least ten practices will undertake training in Health Coaching and receive support to use the knowledge and skills gained when utilising the Lifescripts resources. A Coaching4LIFE resource kit that includes a flow chart showing how health coaching and Lifescripts complement each other, Lifescripts tools, and resources to assist with referrals to local services will be developed.
Central Highlands Division of General Practice & Adelaide Western General Practice Network
These Divisions will assist between ten and twenty practices to focus on lifestyle management for patients with risk factors of unhealthy weight and/or smoking. The Collaboratives model will be used to assist practices collect and use data to identify patients and measure progress. Practices will determine how the targeted patients will be managed and will then be supported by Divisions to implement their chosen strategies.
GP Coastal (Perth Central Coastal Division of General Practice) & Perth and Hills Division of General Practice
This project will take a whole-of-practice approach with the six participating practices. An assessment of each practice’s needs and capabilities will inform individual implementation plans. The Divisions will support the practices by providing education sessions, establishing a network and providing regular contact. The Divisions will also run group sessions for patients on self-management and healthy lifestyles. GP Coastal will be working with the University of WA (Primary Health Care Research Evaluation and Development) team to develop evaluation tools for its project.
Riverina Division of General Practice
This project will utilise multiple strategies to assist practices integrate sustainable lifestyle risk factor management (LRFM) into their core business. The Division will offer one-on-one educational academic detailing to all 37 practices in the catchment and promote LRFM to the broader community. It will provide additional support for 10 practices that will be recruited to an ‘intensive stream’ (four based in Wagga Wagga and six rural). For these practices, the Division will provide regular practice visits to support integration of LRFM and data collection on usage of Lifescripts.
Hunter Rural Division of General Practice
The Division will provide ongoing training and support to five general practices and one Aboriginal Medical Service to increase their utilisation of the Lifescripts resources. The Division will also recruit a ‘champion GP’ to assist with promoting Lifescripts to participating practices. One of the project’s objectives is to compare the effectiveness of a ‘patient-led’ approach versus a ‘GP-led’ approach to increasing Lifescripts uptake. This will be achieved by implementing a media campaign to promote patient requests for Lifescripts in an identified solo practice town and measuring the effect in comparison with other participating practices.
Gold Coast Division of General Practice & South East Alliance of General Practice
The Gold Coast Division of General Pratice will work in partnership with South East Alliance of General Practice and General Practice Queensland to assist general practices to implement preventative health strategies by utilising Lifescripts and the Collaboratives model of quality improvement. The project will utilise two models of quality improvement with 10 practices in each model spread across both Divisions. The target practices will include both those practices new to National Primary Care Collaboratives program and those that have completed it.
Top End Division of General Practice
This project aims to support practices to develop strategies for risk factor management and lifestyle modification with patients. The project will support seven practices in urban, rural and remote locations, including two Aboriginal Medical Services and two NT Government Aboriginal Health Services. The Division will develop two local referral directories (urban and remote versions) and support participating practices though practices visits and the provision of education sessions.
Adelaide Hills Division of General Practice
The Division will engage six practice ‘hubs’, three of which have multiple locations in the region, thus eleven general practice sites will be engaged. The project will support the integration of Lifescripts activities into general practice and enhance the role of the practice nurse through practice visits and the provision of training sessions. Practices will be encouraged to utilise Lifescripts as a part of General Practice Management Plans and Team Care Arrangements to improve patient outcomes.
Central Bayside General Practice Association & Dandenong District Division of General Practice
These Divisions will work with a variety of practices and patient population demographics to develop sustainable and transferable models for the use of Lifescripts within the general practice setting. A total of 14 practices will be engaged across the two Divisions. The project is underpinned by the Collaboratives methodology and will include training and support for practice staff on lifestyle risk factor management, related MBS item numbers and the use of Plan, Do, Study, Act cycles to implement practice change.
