After hours support

The after hours on-call service is designed to provide a service to registered palliative care service patients who require after hours professional support. It is hoped that this will make the lives of those with a terminal illness and their carers more secure and comfortable in the setting of their choice. It is also envisaged that the coordination of palliative care services will improve with regularly up-dated health records available to health care providers and carers.

End of life pathway

A clinical pathway to improve palliative care for inpatients who are expected to die from advanced disease offers a systems-oriented approach to induce organizational change, improve patient access to resources, enhance communication, and directly redress deficiencies in care.

Link nurses

A link nurse group is a group of nurses with an interest in Palliative Care, who would form a resource with a direct link to the palliative care specialist nurses, and hopefully to each other. The role involves attendance at meetings where ideas and new developments can be discussed. Information may then be disseminated from specialist nurses to ward-based nurses, providing formal two-way communication in a clinical setting.
