Rural Palliative Care Resource Kit

Introduction

The Department of Health and Ageing have supported the development of this resource kit through the Rural Palliative Care Program (RPCP).  The RPCP aims to support rural divisions of general practice to develop and implement collaborative models of care that will significantly improve rural community access to timely, quality and coordinated palliative care.

Purpose

The resource kit provides information that can be adapted to assist divisions in the establishment or enhancement of rural palliative care services across Australia.   This site contains documents that will be of assistance to divisions and primary care providers. 

AGPN encourages the use of the resources available and requests that you acknowledge the content source.  A number of the documents are large files and have been saved in portable document format (PDF).  If you would like to adapt any of these documents and require an alternative format contact AGPN via the contacts form on this site.

The resources have been developed by a group of experienced rural divisions of general practice during the pilot phase of the RPCP.  More information on the divisions involved in the RPCP is located at:
