Medication Management Review
The Medication Management Review Facilitator Program (MMR Facilitator Program) is the key national infrastructure to support the implementation of Home Medicines Review (HMR).
Home Medicines Review
The Home Medicines Review (HMR) was introduced into the Medical Benefits Scheme in October 2001 to increase the appropriate use of medications and reduce the incidence of 'Medication Misadventures', thereby assisting in improving patient health outcomes.
The HMR is a consumer-focused, structured and collaborative health care service provided in the community setting, to optimise quality use of medicines and consumer understanding. It involves the consumer, their general practitioner, their pharmacy, and other relevant members of the health care team.
MMR Facilitator Program
The Australian General Practice Network (AGPN) supports the Australian Government's commitment to this initiative. AGPN is also supportive of a continuation of the Medication Management Review Facilitator Program (MMR) that facilitates the uptake of HMR.
The MMR Facilitator Program builds upon the experience and recommendations contained in the evaluation reports of several Medication Review Projects conducted in Australia in recent years.
National and State infrastructure for the Program includes a National Management Group (NMG) and State Facilitators. The Northern Territory and each State Based Organisation will also receive limited funding in recognition of support in assisting the Guild State Branches work effectively with Divisions and GPs.
Divisional Facilitators assist both general practice and pharmacy take up the opportunity that HMRs represent to provide evidence-based, multi-disciplinary and preventative care to consumers.
