Summary of Programs & Projects:
CPD & WSRGP:
- An established CPD program that enables visiting medical specialists to the State health system to provide ongoing CPD to the local workforce.
- Provision of annual subsidy to private GPs to attend CPD outside the Pilbara.
- Development of subsidised clinical attachments to tertiary city hospitals for Private GPs.
- Provision of in-practice support for CDMI, Clinical and Business systems and Accreditation.
- Establishment of credentialled Director Training for PDGP Directors.
Mental Health:
- Integration of Mental health professionals into ongoing CPD with small groups of GPs in 4 different Pilbara towns.
- Establishment of 18-month program of CPD to teams of General Practice, Northwest mental heath and Pilbara Community Drug Services.
- Involvement in Suicide Prevention Network.
- Production of wallet-sized business cards with emergency numbers for youth.
- Establishment of 2 small learning groups providing peer support and education.
Immunisation:
- Continuation of sub-immunised lists to private GPs, community and child health teams. Continuation of fridge audits with follow up recommendations.
- Development of 4-yr old friendly resources as incentive / reward for completion of childhood schedule, shared between state health and general practice.
- Support Practice nurses to attend city-based annual upskilling with rural peers.
Aged Care GP panels:
- Establish a profile of Pilbara services.
- Develop GP Panels.
- Provide education and resources to teams engaged in Aged Care.
Health Promotion:
- Development and production of innovative, youth friendly resources led by local youth and guided by PDGP staff and GPs.
- Promotion of and delivery of Pit Stop Men's Health to inland Pilbara towns and communities.
- Provision of resources to General Practice to concur with Population health's current health promotion activity.
MAHS:
- Provision of team-based services, delivered where possible in General Practice, for people diagnosed with Diabetes. Disciplines include Podiatry, Dietetics and Diabetes education.
- Continuation of Audiology.
- Possible development of counselling services.
Palliative Care:
- Development of a program that enhances palliative care service provision by supplying education, training, resources and knowledge and by collecting and collating data on a regional basis.
- Development and strengthening of volunteer base.
- Awareness raising and information to community members.
Leaping Lizards:
- Development of a program targeting school children's physical activity levels and nutrition knowledge and habits.
- Delivered in schools.
- Provision of CPD to school staff.
- Provision of role-model program to school children.
- Provision of innovative physical activity equipment to schools.
HMR and NPS:
- Provision of Pharmaceutical education and training to General Practice and community health teams.
- Establishment of HMR in 3 Pilbara towns with more to follow.
