Programs and Projects
Summary of Programs & Projects:
The Division has identified several five key areas of focus over the next three years. They are described below as the major program areas of the Division:
- Integration
This includes projects and services such as the GP Collaboration Unit, the Antenatal Shared Care program, the Mental Health Shared Care program, the North Wyong Primary Care Unit and the Central Coast Primary Health Care Network projects.
- Access
This includes the projects and services such as the cervical screening outreach clinic, the Division's Practice Nurse Support Program, The Better Outcomes in Mental Health - Access to Allied Health services project, the After Hours GP Clinic and Aboriginal Health.
- Population Health
This area has been broken down into Chronic Disease Management and includes projects and services such as Asthma Watch, The Central Coast Diabetes Program, the pilot of a Cardio vascular Disease project and participation in the NSW Health Chronic and Complex Care program.
The other area is health prevention and includes projects and services such as Immunisation project and the Home Medications Management project.
- Quality and Evidence
This includes projects and services such as the Quality Use of Medicines project, the Division's CPD and Information Management programs.
- Corporate Governance
This includes all the internal business systems such as human resources, finance, marketing, communications and publications, internal IMIT support, the Board, health service planning and GP and community liaison
