Summary of Programs & Projects:
New services to be introduced to members include:

  1. Tele-coach service - supporting GP clinical management of patients through regular phone contact with Tele-Coach offering support and prompting for patients to comply with clinical management guidelines e.g. asthma 3+ plan, diabetes cycle of care, and healthy lifestyle coaching re: exercise
  2. Clinical resource faxback - GP access to evidence-based clinical management resources and guidelines through on-call clinical management fax-back service
  3. Referral Linkage service - dial in access/referral to specialist and allied health services
  4. Resi-Care Improvement service - GP Residential Care Panel, GP and residential care staff training
  5. After-hours Service - work with local GPs and neighbouring Divisions to establish after-hours primary care service based on GP cooperative model
  6. Clinic Workforce assistance - recruitment and training, position descriptions, industrial relations advice.
  7. Clinic Continual Quality Improvement - assist clinics in cyclical strategic and business planing at an individual clinic level
  8. Existing services to be further developed and marketed to members:
  9. Hospital Liaison service - jointly with neighbouring Divisions and hospital network develop GP-patient referral, discharge & feedback systems.
  10. GP Health & Pastoral Care - focus on GPs' own health is based on a "lead by example" approach.  GPs who undertake healthy lifestyle change are more likely to promote similar activities to their patients
  11. Health Promotion - address local health needs through health promotion on youth health, cardiac health and GPs' own health.  Young people will again be the focus of our GP speaker service as we continue our involvement with the Knox Safe Party project which promotes harm minimisation at local secondary colleges.
