Hawkesbury-Hills Divison of General Practice incorporates the former Hawkesbury Division of General Practice and the Hills district of North-Western Sydney.

Hawkesbury-Hills DGP opperates a practice support model to implement programs in partnership with General Practice. Each practice has a designated Practice Support Officer who is the primary link between the Division of General Practice. Using this model, Hawkesbury-Hills DGP provides support programs targetting Immunisation, Enhanced Primary Care, Diabetes, Aged Care, Mental Health, Nursing in GP, Quality Use of Medicines, Antenatal Shared Care, CPD and Practice Accreditation. The Divison opperate two clinical services - Hawkesbury After Hours GP Service and an Access to Allied Health Services program.

The future of Primary Health Care delivery will see greater integration of General Practice and the wider health system. Supporting this integration is a core activity of Hawkesbury-Hills DGP and facilitating closer links between General Practice and other parts of the health system will remain a key focus of future activity.
