Programs > > For Divisions Division Implementation In December 05, May 06, November 2006, and March 07, all Divisions that have chosen to implement Lifescripts were asked to complete a 'Lifescripts Implementation Data Collection Form'.  The results of the completion of this form by Divisions across Australia were used to compile a National Implementation Profiles at each point in time.  These profiles outlines the variety of methods Divisions have used to implement Lifescripts, and can be found in the 'Division Suppot Resurces' section of the Lifescripts . Further implementation models are outlined in a variety of Divisional Lifescripts Case studies that can also be found in the Lifescripts . Prevention, early intervention and chronic disease management are priority areas in the new national quality and performance system for Divisions of General Practice. Lifescripts is a program that Divisions can participate in to add a preventative and risk reduction element to chronic disease management programs.  Examples of ways Lifescripts may be incorporated into Division annual plans can be found in the Lifescripts

The Wide Bay Division of General Practice works with 169 General Practitioners across the region and is an active partner in a range of primary health initiatives. The Division's program activities are developed through collaboration and networking with other local health service providers and focus on work that general practice cannot feasibly undertake without support.
