Divisions of General Practice Achievement Awards

The aim of the Divisions Achievement Awards is to recognise Divisions for their achievements in everyday Divisional activities. The Divisions Achievement Awards are an opportunity to celebrate excellence and innovation and encourage on-going improvements in Divisions in their delivery of primary care services.
The 2005 Divisions Achievement Awards will be awarded for recent activity (undertaken within the past 2 years) in each of the following categories:
- Improving Division Performance
- Supporting Quality in General Practice
- Improving Access to Primary Health Care
- Promoting Prevention and Early Intervention
- Aboriginal and Torres Strait Islander Health
Special Award Categories:
- Best Division Website
- Best Division Newsletter
Nominations for Divisions Achievement Awards will be judged against the Selection Criteria in each category by a panel of the Forum Steering Committee, which includes representatives from Divisions, SBOs, ADGP, DoHA, Consumers Health Forum and General Practice Registrars Australia.
The Divisions Achievement Awards may be either self-nominated or nominated by an external organisation. Nominations must:
- address each of the Selection Criteria listed below, and
- be supported by testimonials provided by other members of the Divisions Network, and external bodies such as community organisations, District Health Boards, consumers etc.
The Awards will be presented during the Opening Plenary session at the Divisions of General Practice Network Forum on Friday 4 November in Perth.
Nominations close on 1 September 2005. Instructions on how to submit a nomination are below.

Category 1. Improving Division Performance
1.1. Evidence that the described activity has improved the peformance of the Division 1.2. Evidence of a sustained approach to the activity 1.3. Evidence of outcomes achieved by improved Divisional performance in area of activity 1.4. Testimonials
1.4.1. One from within the Divisions Network 1.4.2. Two from organisations external to the Divisions Network
Category 2. Supporting Quality in General Practice
2.1. Evidence that the described Divisional activity has supported quality and evidence-based care within member Practices 2.2. Evidence of a sustained approach to the activity 2.3. Evidence of outcomes achieved through the activity 2.4. Testimonials
2.4.1. One from within the Divisions Network 2.4.2. Two from organisations external to the Divisions Network
Category 3. Access to Primary Health Care
3.1. Evidence that the described Divisional activity has promoted and supported access to primary health care 3.2. Evidence of sustainability of the approach 3.3. Evidence of the promotion and involvement of practice teams in this activity 3.4. Evidence of outcomes achieved through the approach 3.5. Testimonials
3.5.1. One from within the Divisions Network 3.5.2. Two from organisations external to the Divisions Network
Category 4. Prevention and Early Intervention
4.1. Evidence that the described Divisional activity has promoted and prevention and early intervention 4.2. Evidence of sustainability of the approach 4.3. Evidence of the promotion and involvement of practice teams in this activity 4.4. Evidence of outcomes achieved through improved health outcomes within the target group 4.5. Testimonials
4.5.1. Two from non-Board GP members 4.5.2. One from a practice manager of a member practice
Category 5. Aboriginal and Torres Strait Islander Health
5.1. Evidence of improved service delivery resulting from a collaborative activity or partnership 5.2. Evidence of sustainability of the partnership or activity 5.3. Evidence of the promotion and involvement of practice teams in this activity 5.4. Evidence of health outcomes achieved within the ATSI population through the activity or partnership 5.5. Testimonials
5.5.1. One from within the Divisions Network 5.5.2. Two from organisations external to the Divisions Network, at least one of which is from a participating organisation in the collaboration or partnership
Special Award Categories
Category 7: Best Division Website 7.1 Evidence of consistent use of the site by the website target audience (statistics, enquiries, testimonials) 7.2 Evidence of quality and best practice in website design - usability of navigation, look and feel, opportunity for interaction by users 7.3 Evidence of quality in website content - high level currency, relevance, readability, comprehensiveness 7.4 Evidence of methods used to promote use of the site by target audience/s and evidence of ongoing development/improvement of the site
Category 8: Best Division Newsletter
8.1 Evidence of a successful and/or innovative approaches to dissemination, design or development of the Division's newsletter 8.2 Evidence of quality and best practice in newsletter design - layout, navigation, effective use of images and graphics 8.3 Evidence of quality in Newsletter content - high level of currency, relevance, readability and comprehensiveness 8.4 Evidence of methods used to maximise newsletter readership and to promote its effectiveness as a communication tool to the target audience/s
All Division Achievement Awards should be made on-line.
During the on-line process, you will be prompted to upload a Word document containing your nomination details - this document must address each of the Selection Criteria for the category you are nominating for, and include 3 testimonials.
