The Getting Patients Treated Program
The Getting Patients Treated Program for elective surgery management reform is an innovative response to elective surgery waiting lists within the tertiary hospitals. In this Program, General Practitioners, through their Divisions, review patients on waiting lists for treatment at non-tertiary hospitals or by General Practitioner proceduralists. The program also provides for an increase of General Practitioner proceduralists within the community, and an enhancement of information technology within General Practice to facilitate information sharing with the Central Wait List Bureau.
HomeWard 2000
HomeWard 2000 is a collaborative General Practice Demonstration Program, which provides comprehensive acute hospital-in-the-home care for patients who would otherwise require hospitalisation. The program provides up to seven days of acute home care, with care management coordinated by the patient's own GP or by one of a team Acute Care GPs who provide 24-hour on-call to the Program. Nursing services are provided through Access Home Care, with ancillary services provided as necessary.
Enhanced Primary Care
The Enhanced Primary Care package is made up of a range of programs designed to assist people with chronic illness and complex care needs (many of whom are older Australians) as well as their carers and the health professionals who look after them. As the State-based organisation for Divisions, GPDWA coordinates the Statewide Education and Training Strategy for the new MBS items, in liaison with the 15 Divisions of General Practice, and also facilitates strategic discussion regarding the multi-disciplinary opportunities offered by the new MBS items.
GPDWA works closely with the State Office, Commonwealth Department of Health and Aged Care, the Health Department of Western Australia, and a range of stakeholders who are represented, or are in communication with, the EPC Strategy Steering Group, which is convened by GPDWA.
Primary Mental Health Care
The Primary Mental Health Care Initiative is a component of the National Mental Health Strategy, which aims to provide GPs with education in mental health care, and to improve links between GPs and specialist mental health services (public, private and non-government). Within Western Australia, a State-wide Mental Health Steering Group has been established, with representation from rural and urban Divisions, consumers, the Health Department of Western Australia, and the State Office, Commonwealth Department of Health and Aged Care. In response to the initiative, WA Divisions are working to enhance collaborative relationships with local health services, and to bring together stakeholders to explore innovative responses to the challenges of local mental health service provision. More recently, a Mental Health Taskforce has been established between GPDWA, the Health Department of Western Australia, and the State Office, Department of Health and Aged Care, to explore system responses to mental health service delivery.
