CLIMATE and ADGP invite YOU to attend this ADGP Forum breakfast meeting to hear about an exiting chronic disease management programme.  Registration closes Friday 17 September

Who should attend?   GPs, Division and practice staff concerned about chronic disease management.

What will be presented?  Information about a new internet based patient education system to simplify chronic disease management.

When?    Friday 24 September at 7.15 - 8.30 am

Where?  Riverbank Rooms 2/3 in the Adelaide Convention Centre

How will it be presented?  There will be presentations from:
