Divisions Role in New Item Numbers
Divisions have a key role in promoting and supporting structured, quality multidisciplinary care for better chronic disease health outcomes. Uptake of the new CDM items by general practice is important to both the quality of chronic disease management and to the general practice setting contributing to national primary health care priorities.
ADGP has been funded by the Australian Government Department of Health and Ageing to appoint a National Chronic Disease Coordinator to develop education and training strategies to raise awareness of the item numbers and promote uptake. The National Coordinator will be appointed for a period of six months. The National Coordinator will consult with State Based Organisations and Divisions about General Practice and the Networks information, education and training requirements and will develop a suite of downloadable resources such as flowcharts, check lists and frequently asked questions. The resources will be uploaded onto ADGPs website as they become available.
