Practice Capacity is about the organisation, systems and ways of working that practices need to provide high quality care for patients with chronic illness. Research shows that improvements in the organisation of a practice enhances the quality of care provided for chronically ill people. In particularly the following:

Clinical information systems such as disease registers, reminders, feedback;

Decision support systems;

Multi-disciplinary teams;

Linkages with community referral services and resources;

Information management and data exchange; and

Patient self-management support.
