Summary of Programs & Projects:
Primary Prevention & Health Promotion
- Supporting practices to address chronic disease
- Activity-SNAP and provide lifestyle advice to patients
- Enhancing awareness of local service providers who can assist patients with lifestyle/risk factor modification
- Promoting evidence based practice in screening for diabetes, cervical cancer etc
- Maintaining and improving the Division's immunisation rate.
Disease Management and Secondary Prevention
- Enhancing practice capacity for best practice, effective and planned care for patients with chronic diseases
- Enhancing awareness of local service providers, and promoting multi-disciplinary care for patients with chronic diseases
- Providing additional mental health allied health services in high-need areas through the Better Outcomes in Mental Health Care (BOMHC) allied health project
Quality in General Practice
- Promoting and supporting practice accreditation
- Participating in the CALM (Conflict and Litigation Minimisation) risk management program
- Providing education and quality improvement activities for GPs and practice staff, including the promotion & establishment of small group learning across geographic regions
- Providing local health needs & service delivery data to practices for the purposes of planning and quality improvement
- Providing NPS Quality Use of Medicines activities
- Promoting collaboration between pharmacists and GPs through the use of Home Medicines Reviews
Access to Primary Care
- Supporting GPs and local aged care facilities to improve the quality of health care for residents of aged care
- Facilities
- Supporting GPs and other local health services to improve the quality of health care for young people
- Undertaking a trial of alternative After Hours services in the Macedon Ranges region
- Providing MAHS (More Allied Health Services) services in rural Community Health Services throughout the
- Division
- Supporting the recruitment and retention of suitable trainees, registrars, oversees trained doctors and other GPs throughout the Division area
