Brisbane Inner South Division of General Practice is a small urban Division with 210 active GPs and approximately 400 practice staff in 56 practices. The Divisional Team consists of 11 staff members (5 of whom work across two Divisions - Brisbane Inner South and Bayside). The Division is co-located with the Brisbane South Centre for Health Service Integration and is one of the HealthConnect pilot sites.

Challenges for the Future:
BISDGP believes that in order to positively impact on the care provided by GPs to patients, GPs need to be supported via a variety of mechanisms to improve their practice processes. To this end, BISDGP is utilising the model promoted by Wagner et al of organising effective care for people with chronic illness. The core components of this model apply across all aspects of General Practice. They are:

  • Practice redesign - appointments, follo-up, and recall.
  • Patient education - self-management, behavioural change, patient participation.
  • Expert support systems - provider education, decision support.
  • Information or data - reminders, outcomes, feedback and care planning.
