Medication Management Review
The Medication Management Review Facilitator Scheme (MMR Facilitator Scheme) is the key national infrastructure to support the implementation of Domiciliary Medication Management Reviews (DMMR).
The DMMR is a consumer-focused, structured and collaborative health care service provided in the community setting, to optimise quality use of medicines and consumer understanding. It involves the consumer, their general practitioner, their pharmacy, and other relevant members of the health care team.
The MMR Facilitator Scheme builds upon the experience and recommendations contained in the evaluation reports of several Medication Review Projects conducted in Australia in recent years.
To promote quality uptake of DMMRs, a part time MMR Facilitator has been employed in each of the 121 Divisions of General Practice, who sub-contract to the Pharmacy Guild of Australia (Guild) to gain funds from the 3rd Pharmacy Agreement between the Guild and the Department of Health and Ageing (DoHA). The Scheme is therefore a joint program between the Guild and Divisions and cooperation between the Guild and Divisions at National, State and local levels will be a crucial factor in the successful implementation of the MMR Facilitator Scheme.
National and State infrastructure for the Scheme includes a National Management Group (NMG), National Managers and State Facilitators. The Northern Territory and each State Based Organisation will also receive limited funding in recognition of support in assisting the Guild State Branches work effectively with Divisions and GPs.
