A one-day Forum will be held on 15 April 2005 to profile the research outcomes from the 'Practice Capacity for Chronic Disease Care' study conducted by the Centre for GP Integration Studies/University of Adelaide and to highlight other advances in the field.

This will be particularly relevant to the new Division's contracts and performance indicators, and for the National Chronic Disease Strategy.

Target Audience:

• Divisions, SBOs, standards and accreditation
  agencies and other organisations supporting general practices
• GPs, practice staff and other health professionals involved in chronic disease management
• Policy makers, national and state program staff

Why: Join with colleagues to discuss the organization and capacity that practices need for chronic disease care and how this can be developed, based on recent research, experience in the field and current national programs, particularly the National Chronic Disease Strategy.
