HeartBeat is a community-based program
for patients following hospitalisation at Rockingham Kwinana
District Hospital as a result of a cardiac event. It focuses
on the GP in the role of care coordinator with individualized
care plans being developed for patients. It also focuses on
diabetes intervention. |
