Integration of EPC into established Mental Health programs."A
Model for Sharing Mental Health Care in Far North Queensland"
has been developed which incorporates suggested referral pathways
and options for shared care. Resources have been developed
for GPs and Mental Health Services include MH Case Conferencing
Form, Mental Health Care Plan/Case Conference Checklist and
patient consent forms.
