CHECKLISTWould you like a GP to participate in a discharge case conference for a patient with complex or chronic multidisciplinary needs?The focus of discharge planning is the patient's usual location (home, school, community etc.) and as such will often be enhanced by involvement of the patient's GP in the care management planning and review. This is especially important for patients with complex needs (eg presence of complex medical, surgical or disability issues and/or complex social circumstances). |

Software:
Eyre Care Software
Eyre Peninsula Division had done considerable work towards designing software that supported the new EPC items and in doing so, were saving heaps of time! In the meantime, ADGP became aware of the national need for software to support GPs to enable them to incorporate EPC into their electronic practice. Pfizer not only had the capacity to develop the software but agreed to do so as an initiative to support Divisions of General Practice and their GPs.
