- You are able to contribute your services as part of a team
approach to care
- The case conference allows your participation towards an agreed
objective in consistent care
- Your role and responsibility is discussed in the case conference
� A summary of the case conference provides a reference point
for team members
- The GP is aware of your expertise and contribution
- A link to the patient's GP is formalised
- Consistent and well informed care is being provided to the patient

If you are not a GP and wish to initiate a case
conference
- Many health care providers already hold case conferences, for
example, with case management of patients
- The new Medicare Benefits Schedule supports a GP to participate
by contributing to a case conference convened by another health
care provider
- The patient's usual GP will need to be contacted for agreement
for contribution

Documentation
- The GP will need to place a copy of the case conference on
the patient's file
