Participant in Combined Southside Divisions Discharge Planning
project involving QE II Hospital. Based on consultation with
GPs and Hospital and Community staff, pathway established
to facilitate GP participation in discharge case conferences
and care plans from QEII Hospitals' Rehabilitation Ward. Documentation
developed including information leaflets for GPs, Practices
and Hospital staff.
